Dial the following USSD code from mobile which phone’s SIM card you want to block.
As soon as you enter this USSD code, it will proactively ask you to enter PUK (Personal Unlock Key) code. You must know PUK code of that sim card you want to block.
Interesting fact: If you enter the wrong PIN code of your sim card more than 3 times, it will ask to enter the PUK code to reset the PIN. And, if you enter the wrong PUK code more than 10 times, it will also block your sim card forever.

After some telephonic verification, they will block your sim card, most probably.
You can contact cellular mobile service providers in India by just calling a toll-free number 198 from any of your GSM/CDMA mobiles.
198 is a PAN-India number for registering complaints and requests only. Keep in mind that there is some daily call limitation to 198 for each Indian telecom operators, meaning you call may not get connected.
