Simply, dial this USSD code on that mobile which phone’s SIM card you want to block.
As soon as you enter this USSD code, it will proactively ask you to enter PUK (Personal Unlock Key) code.
You must know PUK code of that sim card you want to block.
In case you don’t know PUK, just contact your telecom service operator to get the PUK code.
The above secret code to block sim card is working fine with all major telecom companies in India including AirTel Cellular, Vodafone, Idea Cellular, BSNL, MTNL, Tata Indicom, Reliance, MTS, etc.
NOTE: Entering wrong PUK code more than 10 times, would block your SIM card permanent and could not be reactivated later on.
Contact Your Telecom Service Provider to Block SIM Card Temporarily

Simply call the customer care representative and tell them that you have lost your sim card and want to block incoming and outgoing immediately.
After some telephonic verification, they will block your sim card, most probably.
You can contact cellular mobile service providers in India by just calling a toll-free number 198 from any of your GSM/CDMA mobiles.
Basically, PAN-India number 198 is for registering complaints and requests only.
There is also some daily call limitation to 198 for each Indian telecom operators.
